Program Guidelines
The Preferred Plan is an annual benefit option offered by Edgeview Dental PLLC. I understand this plan is not dental insurance, but a discount plan optioned to those patients who do not have the benefit of personal dental insurance.
Membership is purchased annually from the original start date. All payments must be made in full either cash, by personal check or credit card on the date services are provided in order to receive the discount. Care Credit cannot be used as a form of payment. The plan may be cancelled at any time or the discount waived if member does not meet the financial agreement.
